APA Supervisor Annual Performance Narrative Form
Annual Self-Report (Appendix C) must be submitted by all APA administrators on or about November 1 for the new fall-based annual review schedule. This reflective document should:
Articulate key accomplishments and outcomes for the past year;Identify progress toward strategic and departmental goals;
Acknowledge significant challenges or barriers;
Propose professional goals for the upcoming year; and
Document completed or planned professional development activities
This self-report will serve as the foundation for your supervisor’s performance evaluation.
