Medical Holds

Before the fall and spring semesters are in session, all incoming freshmen and transfer students need to pay an initial deposit. Once the University has received the deposit, students will receive information from Admissions regarding the health requirements and the link to the university health form.

Medical requirements are mandated by the state of Massachusetts.

All new students must have their up-to-date medical records in by the due date: July 15th for students entering in the fall and December 15th for students entering in the spring.

If health records are not in by October 1st for students entering in the fall, and February 1st for students entering in the spring, the student is put on a medical hold. This means that the student will not be able to register for the classes they plan to take the following semester. Students are kept on hold until all required medical information is received by Health Services.

Before holds are put on (about a full month before the registration period), all students are notified via student email (@student.fitchburgstate.edu) encouraging them to check their Web4 to make sure there has not been a hold applied to their account.

  • Please read Web4 carefully. If there is a medical hold, you need to call Student Health Services at 978.665.3643/3216 and find out what you are missing. Any missing information can be handed in by the student at Student Health Services or faxed in by the student's primary care provider. Please attend to this – if a hold does not appear on Web4, you have a complete medical record.

Please refer to our health requirements for a complete list of mandatory medical information needed by Health Services for your records to be complete.

Make it a practice to consistently check your Fitchburg State email and Web4.
