Welcome to Fitchburg State University!

Your health is very important to your academic success, and we at Health Services are available to provide you care for your medical concerns. If we need to refer you to a local physician, we can assist you in locating one. The American medical system can be quite different from what you are accustomed to at home, so we can assist you in obtaining the care you need.

Medical Record Checklist

All international students are required to submit the correct medical records (PDF) to Student Health Services on the first day of orientation.

Please carefully review the last page, Tuberculosis Risk Assessment, and if you answer “yes” to any of the three questions, you will need to have a tuberculosis test done in the United States, not in your country of origin. These tuberculosis tests will be performed the first day of orientation before you begin classes. The cost of the test is $81.96, so please bring the money with you to pay for the test. We do not accept credit card payments. Your failure to submit these records will result in your inability to register for classes.

All of the listed vaccines are required by law. Please bring proof of all of the vaccines that you have previously received. The University health insurance plan will cover the costs of vaccines that you still need, except for the tuberculosis test.

Transfer Students

Students transferring from other U.S. institutions should be aware that vaccination requirements are not uniform among all U.S. colleges and universities. As such, even if you have met the vaccination requirements at your current campus, you might not meet the health requirements for Fitchburg State.

Please examine Fitchburg State's requirements carefully to determine whether you should seek additional immunizations from your current health care provider. A copy of your current campus health record is mandatory in meeting our health records requirements upon your arrival here. Transfer students should consider providing an Authorization for Release of Medical Records (MS Word) to your current campus health services office, so that your current records will be transferred to our Health Services office here for review. Be sure to initiate this process in time for your records to reach our Health Services office prior to Orientation.

Health Insurance

The United States does not offer universal health care. All international students with an F-1 VISA will be enrolled by the University and will be responsible for the fee.

The effective dates for the insurance is from August 1, 2018 to July 31, 2019. The full year rate is $2985. Fall only 08/01/2018 to 12/31/2018 $1,253.
