Streaming Media Request Form

Eligibility: Streaming media hosting is available to any faculty or staff member. Any media must be used for academic or administrative business.

Requests: Requests to host a file must be made via the online request form. Information Technology reserves the right to turn down requests that do not meet department requirements or University policy.

Content: File owners are responsible for ensuring that the material contained in the media files conforms to applicable University policies.

Hosting Limitations: Information Technology reserves the right to limit media file size and/or hosting duration on a case-by-case basis.

Acceptable Formats: CD, DVD, VHS, m4v, mpeg, mp3, mov, and m4a

Copyright: Materials must adhere to applicable copyright laws and the Teach Act. If you have questions about fair use of copyrighted materials in the instance in which you are making this request, please contact the Copyright Liaison, Connie Strittmatter, at The liaison is available to answer questions about applicable copyright law, and fair use, and to help faculty who may need to seek permission from license holders. Notification: Notification that the media has been posted will be done via email and the Help Desk ticket system.

Turnaround Time: Information Technology will upload typical requests (three or fewer files) within two weeks of receipt of the file(s). Requests for more than three videos will require special arrangements.

Changes to the Policy: Information Technology reserves the right to change these requirements at any time, without notice, if resource or performance issues arise.
