Health Requirements

During the coronavirus pandemic, we understand that receiving a new physical or your health forms being completed by your doctor may not be possible. We ask that you obtain your most recent physical and immunizations even if they are more than one year old. Please fill out the first two pages of the health forms and attach your most recent physical and immunizations. Due to the ongoing COVID-19 situation, we are also unable to accept forms in person or fax. Please submit all documentation through your Application Status Page and upload under Health Forms. The University requires a completed health form and the following state required immunizations be submitted to Health Services for all full time students and all international students both undergraduate and graduate. If you are entering the University in September you need to return the completed form by July 15. If you are entering the University for the January term then you need to submit the completed form by December 15.

  • Complete the Fitchburg State Health Form (PDF), including a physical exam that has been completed within the past year.
  • If you choose to submit health forms from your medical provider, please submit the first page of the Fitchburg State health forms, to provide us with your emergency contact information.
  • Two (2) doses of MMR vaccine or a positive blood titer
  • One dose of TDAP vaccine within the past 10 years
  • Three doses of Hepatitis B vaccine or a positive blood titer
  • MenACWY (formerly MCV4) meningococcal vaccine or a signed waiver Meningitis Waiver Form (PDF)
  • Complete the Tuberculosis risk assessment form, which is a part of the Fitchburg State Health Form (PDF). Circle the country or countries below where you have lived for the past 6 months. If you answer "yes" to any of the three questions, you will need to have a T-SPOT Tuberculin test or a PPD tuberculin test. University policy states that PPD tests must be done in the USA.
  • Two varicella vaccines for chickenpox or a documented history of the disease or a positive blood titer.

Failure to comply with these requirements may prohibit you from registering for future classes.

If you are currently in the military or were previously in the military please notify us immediately. We will also accept your DD 214 form.

Health Insurance Requirement

All international students with an F-1 VISA are required to enroll in the Fitchburg State University Student Health Insurance Plan. This policy is effective from 9/1/2019 to 7/31/2020 and the price for the full year is $3,444. For Fall Semester only (08/01/2019 to 12/31/2019), the price is $1,444.
