2012 Graduation Health Review

HEALTH SERVICES CONGRATULATES YOU ON YOUR ANTICIPATED GRADUATION!
This information will help you transition from Fitchburg State Health Services to the health care system of your choice. Please read this carefully and take care of as much as possible during your final semester on campus.
APPOINTMENTS CAN BE MADE BY CALLING (978) 665-3216 / 3643:
During the summer, Health Services is open for administrative issues only. Once you graduate, you will be unable to utilize Health Services. This may be the first time in your life that you are responsible for securing a health care provider, health insurance, and health care on your own. Don’t be caught uninformed or uninsured. We have enjoyed being your access to health care while at Fitchburg State University and want to wish you a very health future.
If you have utilized Health Services and need a prescription refilled, including birth control, please make an appointment at Health Services as soon as possible.
Female Students that need to schedule a final GYN Exam must do so.
Contact Health Services for a copy of your medical or immunization records. The first copy is free. You also need to check on your present health insurance coverage. Many family plans end when you graduate.
Immunization and Medical Records
Health Services can furnish you with a copy of your immunization records. You may want to have an official copy for your personal records, future graduate school, or job requirements.
Your complete medical record of visits and treatments at Health Services are also available to you. Both the immunization and medical record are confidential, and will not be released to anyone unless you sign a HIPAA “Medical Record Release” form. These forms are available at Health Services or on line at www.fitchburgstate.edu/campus-life/student-services/health-services/ . Student records are kept for ten years after graduation and are then destroyed.
HEALTH INSURANCE
While a full-time student at Fitchburg State University, Massachusetts law required that you have health insurance. It is important to find out the termination date of your present health insurance policy.
If you are presently on FSU Student Health Insurance Plan offered by Blue Cross Blue Shield, the plan terminates on: 12:01 a.m. August 1, 2012.*
If you are presently on your parent’s family policy, your enrollment may end after graduation. Many policies will only let you be on your parent’s family plan while you are a full-time college student. Call your insurance company and find out when you are terminated from your parent’s plan.
